Job summary

BAYADA Home Health Care is seeking a Physical Therapist for per diem, part-time or full-time home health visits in Bushkill and surrounding areas. The PT will join a multidisciplinary team to provide rehabilitative care and skilled nursing in the client’s home.

Requirements include a PA PT license and at least one year of PT experience, with strong communication and time-management skills. EMR experience is preferred and field-based work is expected.

Qualifications

• Active Pennsylvania Physical Therapist license.
• Minimum of 1 year of PT experience.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient with electronic medical records (EMR) is preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform mobility, strength, balance, and safety assessments.
• Implement wound care and protective interventions using evidence-based dressings.
• Develop and revise individualized care plans including therapeutic exercises and gait training.
• Educate clients and families on rehabilitative care and activities for safety and independence.
• Document observations and interventions accurately using a touch-pad device.
