Job summary

BAYADA Home Health Care seeks an experienced Physical Therapist (PT) for a full-time role providing home health visits in Philadelphia. A Pennsylvania PT license and at least one year of clinical experience are required.

Qualifications

• PA PT license required.
• Minimum 1 year PT experience.
• Graduation from an APTA/AMA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements on mobility, balance, gait, strength, and safety.
• Implement wound care and prevention using approved dressings and agents.
• Develop and implement individualized care plans including manual therapy and gait training.
• Revise care plans in collaboration with nursing as part of a multidisciplinary team.
• Educate clients and families in rehabilitative care for health, safety, and independent living.
• Accurately document observations and interventions using an EMR system.
