Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist for a Per Diem, Part-Time, or Full-Time opportunity to provide home health visits across Berks County. A PA PT license and at least one year of experience are required.

You will assess, plan, and implement rehabilitative care, educate families, and document care using electronic medical records while collaborating with a multidisciplinary team.

Qualifications

• A current Pennsylvania Physical Therapist license.
• A minimum of one year of clinical PT experience.
• Graduation from a CAPTE-approved program with transcript or diploma.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and mobility/strength assessments.
• Develop and modify individualized care plans and interventions.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions in EMR per protocol.
