Job summary

Frontpoint Health is looking for a Home Health - PRN - Per Visit Occupational Therapist in Sunnyvale, TX. The role involves providing direct patient care to home-bound patients, developing and revising treatment plans, and working closely with other healthcare professionals to ensure comprehensive care. The ideal candidate will hold a current Texas OT License and be willing to work in the surrounding areas. Responsibilities include evaluating outcomes of care and planning for discharge.

Qualifications

• Must be willing to work in Mesquite, Sunnyvale, Garland, and East Dallas areas.
• Hold a current Texas OT License.
• Possess current Certification in BSL/CPR.
• Have a valid Texas Driver's License and Automobile Insurance.
• Knowledge of Home Care Home Base software is a plus.

Responsibilities

• Develops and revises plan of care with physician and team.
• Provides therapeutic treatments and evaluates equipment needs.
• Participates in clinical conferences and in-service programs.
• Conducts on-site supervisory visits of therapy assistants.
• Advises families and agency personnel as applicable.
• Prepares clinical and progress notes.
• Protects patient and coworker confidentiality.
• Evaluates outcomes of care.
• Plans for discharge.

Skills

Current Texas OT License
