Qualifications

• Must be willing to work in Mesquite, Sunnyvale, Garland, and East Dallas areas.
• Hold a current Texas OT License.
• Possess current Certification in BSL/CPR.
• Have a valid Texas Driver's License and Automobile Insurance.
• Knowledge of Home Care Home Base software is a plus.

Responsibilities

• Develops and revises plan of care with physician and team.
• Provides therapeutic treatments and evaluates equipment needs.
• Participates in clinical conferences and in-service programs.
• Conducts on-site supervisory visits of therapy assistants.
• Advises families and agency personnel as applicable.
• Prepares clinical and progress notes.
• Protects patient and coworker confidentiality.
• Evaluates outcomes of care.
• Plans for discharge.

Skills

Home Health - PRN - Per Visit Occupational Therapist

Job Summary

The Home Health - Occupational Therapist will be responsible for providing direct patient care to home bound patients. The Therapist will work with patients to develop treatment plans, monitor progress, and adjust treatment as needed and helping them achieve greater independence and improve their quality of life. The Occupational Therapist will also work closely with other healthcare professionals to ensure that patients receive comprehensive care.

Responsibilities

• Develops and revises plan of care in consultation with physician and other care-team members.
• Provides therapeutic treatments and evaluation of equipment needs as applicable.
• Participates in clinical conferences and in-service programs.
• Conducts on-site supervisory visits of certified occupational therapy assistants.
• Advises and consults with the family and other agency personnel as applicable.
• Prepares clinical and progress notes.
• Protects and honors patient and coworker confidentiality and right to privacy.
• Evaluates outcomes of care.
• Plans for discharge.
