Job summary

Crossroads Home Health and Hospice in Vallejo, CA seeks an experienced Occupational Therapist to support patients in Solano County. You will evaluate and administer OT plans of care and collaborate with physicians and the care team.

The ideal candidate holds an active OT license, completed an accredited OT program, and has at least 1 year of clinical experience. A valid driver’s license and auto insurance are required; home health experience is preferred.

Qualifications

• Current Occupational Therapy license is required.
• Graduate of an OT curriculum accredited jointly by the AMA and AOTA.
• 1 year of clinical experience.
• Valid driver's license and auto insurance.
• Must undergo and pass a background check.
• Home Health experience preferred but not required.

Responsibilities

• Provide therapy services per treatment plan and care plan.
• Observe and evaluate treatment effects and patient response.
• Update treatment plan as orders change.
• Assess rehabilitative status and develop individual plans.
• Set realistic, necessary goals and care.
• Participate in interdisciplinary planning, conferences and meetings.
• Communicate changes with physician and care team; report significant changes.
