Job summary

UnitedHealth Group's Caretenders is seeking an Occupational Therapist to assess and address patients' functional needs in home settings as part of our post-acute care team.

You will evaluate daily living activities, coordinate with physicians and nurses, and educate patients and families on therapy plans. A valid OT license and CPR are required, plus reliable transportation and a commitment to equitable care.

Qualifications

• Current Occupational Therapy licensure in state of practice
• Current CPR certification
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
• Demonstrate ability to manage multiple tasks simultaneously
• Demonstrate ability to work independently
• Good communication, writing, and organizational skills

Responsibilities

• Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care.
• Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.
• Instructs patient, family, caregiver, and participating members of the health care team in the areas of therapy in which they can participate to assist the patient
