Job summary

UnitedHealth Group seeks an Occupational Therapist for Home Health to assess and address functional status, daily living activities, fine motor skills, home assessments, and adaptive equipment needs in patients’ homes.

Responsibilities include evaluating functional status, collaborating with physicians on care plans, and documenting CMS-related information while working with a multidisciplinary team. A valid OT license and reliable transportation are required.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• Demonstrated ability to manage multiple tasks simultaneously.
• Demonstrated ability to work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, per plan of care, using evidence-based techniques, in coordination with the health care team.
• Evaluates the patient's functional status and CMS documentation; consults with the physician in developing the plan of care.
• Observes, records, and reports to the nurse supervisor and physician the patient’s response to treatment and changes in condition.
• Conducts patient assessments evaluating level of function using diagnostic and prognostic tests.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
