Job summary

Assured Home Health, part of the LHC Group and within the Optum family, is seeking an Occupational Therapist in Home Health. You will assess patient care needs related to functional status, ADLs, fine motor skills, home assessments, and adaptive equipment per medical conditions.

Primary responsibilities include evaluating functional status, collaborating on therapy plans with physicians, and documenting patient progress while maintaining high standards of care across diverse home environments.

Qualifications

• Current Occupational Therapy licensure in the state of Washington.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Provides service within the scope of practice as defined by state laws and in accordance with the plan of care.
• Evaluates functional status and CMS-required documentation; collaborates with physician on therapy plan of care.
• Observes, records, and reports patient responses and changes to condition to nurse supervisor and physician.
• Conducts patient assessments evaluating functional level using diagnostic and prognostic tests.
• Demonstrates ability to work with diverse clients in various home situations.
