Job summary

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides OT services within scope of practice, per plan of care, using evidence-based techniques, coordinating with health care team.
• Evaluates functional status and CMS documentation as required; consults with physician on care plan.
• Observes, records, and reports patient responses to treatment to nurse supervisor/physician.
• Conducts patient assessments evaluating function using diagnostic and prognostic tests.
