Qualifications

• Must have CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Provides service within the scope of practice per state laws and plan of care.
• Evaluates patient functional status and CMS documentation needs; consults with physician.
• Observes, records, and reports patient responses to treatment and changes to condition.
• Conducts patient assessments of functional ability.
• Function in home settings regardless of patient demographics.
