Enhabit Home Health & Hospice is seeking a qualified caregiver to provide skilled home health services under physician orders. The role involves delivering care to patients in their homes, teaching families, and following the established plan of care.

Candidates must be a graduate of an approved school of clinical education, licensed to practice in the state, with at least one year of experience and basic technology skills. Valid driver's license required.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
