Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to provide skilled care to patients in the home. The role involves administering intermittent services, educating patients and families, and following physician orders per the established plan of care.

Candidates should hold a valid license in their state, have at least one year of licenced professional experience, and demonstrate basic technology skills including mobile device use.

Qualifications

• Must be graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
