Job summary

Enhabit Home Health & Hospice is seeking qualified healthcare professionals to administer skilled care to patients at home, in accordance with physician orders and care plans. The role includes teaching patients and families, and collaborating with the health care team to ensure quality outcomes.

Candidates must be graduates of an approved clinical education program, licensed in their state, with at least one year of licensed professional experience and basic tech skills, including mobile device

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
