Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to administer skilled care to patients in home settings. You will educate patients and families and collaborate with a health care team to follow the physician’s orders and the plan of care.

The role requires state licensure, at least one year of professional practice, and strong technology skills for mobile devices. A reliable vehicle and CPR certification are often required.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
