Job summary

The role requires CT RN licensure with at least one year of acute/sub-acute or home health experience, CPR certification, and a valid driver’s license.

Qualifications

• Must be a CT-licensed RN with at least 1 year of nursing experience in acute/sub-acute or home health settings.
• Infusion therapy education completion is preferred.
• Current CPR certification from AHA or Red Cross.
• Valid driver’s license and own vehicle are required.

Responsibilities

• Manages the plan of care and services of assigned patients.
• Coordinates all disciplines involved in the plan of care.
• Partners with others to achieve positive patient outcomes.
• Provides direct care, health counseling, and teaching to agency patients.
• Participates in the Agency’s Community Wellness Program.
• On-call and extended hours as needed.
• Reports to the Clinical Manager.
