Job summary

All About You Collaborative Health Care Services, LLC is seeking a Full Time RN Case Manager to join our team serving Naugatuck and surrounding towns. The RN Case Manager will conduct initial evaluations, develop and implement care plans, coordinate services, and supervise aides in patients’ homes or Assisted Living Facilities.

Qualifications

• RN license in Connecticut (state licensed).
• Two years nursing experience preferred.
• Ability to exercise clinical judgment and communicate with care teams.

Responsibilities

• Provide skilled nursing care in patient homes or ALFs.
• Develop and evaluate the plan of care with the physician and team.
• Coordinate care and supervise home health aides as needed.
• Document visits and patient changes in the EMR accurately.
