Job summary

Qualifications

• Certified Nurse Practitioner or Certified Physician Assistant.
• Active and unrestricted license in the practicing state.
• Current DEA licensure or ability to obtain post-hire as required by state regulations.

Responsibilities

• Provide periodic or intermittent medical care to Optum members.
• Perform comprehensive assessments and document findings per CMS requirements.
• Coordinate care across facilities, families, and physicians to implement plan of care.
• Conduct medication reviews and ensure proper documentation and coding.
