Job summary

Holy Cross Home Care and Hospice (Maryland) is seeking a Home Health Medical Social Worker (MSW) to provide compassionate, patient-centered support in patients' homes. You’ll assess psychosocial needs, connect patients with community resources, support care planning and advocate for goals that promote dignity, stability and independence.

Qualifications

Master's degree in social work from a CSWE-accredited program.
Maryland Social Work License is required.
Minimum two years of social work experience in healthcare settings.
Ability to provide service to acutely and chronically ill patients.
