Job summary

Department of Health and Social Services (DHSS) in Delaware seeks a Health/Human Services Case Manager Supervisor for the Newark office. You will oversee Case Managers and clerical staff, coordinating counseling options, person-centered care planning, and access to social services across aging and disability networks statewide.

As supervisor, you will ensure compliance with laws, write grants when awarded, and manage budgets while reporting on program outcomes.

Qualifications

• Three years' experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
• Three years' experience in making recommendations as part of a client's service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
• Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures.
• Six months' experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.
• Six months' experience in narrative report writing.
• Coursework, training, and/or work experience in staff supervision, which may include performing as a lead worker overseeing the work of others.

Responsibilities

• Plan, assign, review, evaluate, coaching, training, and recommending hire, termination, and promotion for staff.
• Participate in program planning and coordination to ensure compliance with laws, rules, standards, and procedures.
• Gathers and analyzes statistics to ensure program compliance.
• Coordinate with health and human service staff, agencies, providers, and resources to deliver services.
• Represent the agency on task forces, committees, and with the public.
• Explain programs and policies to clients and community representatives.
• May develop grants when awarded, including writing proposals and completing required documents.
• Analyze budget needs and monitor expenditures.
• Prepare statistical and program evaluation reports.
• Utilize information systems to research, review, and report data.
• Communicate effectively across cultures and backgrounds.
