Responsibilities

• Conduct face-to-face and telephone outreach to members regarding preventive health services and social needs.
• Educate members on preventive healthcare and available benefits and services.
• Assess needs and refer to providers, care management, and community resources.
• Establish relationships with members, providers, and community organizations.
• Coordinate access to resources like WIC, Head Start, shelters and social services.
• Document outreach activities, referrals, and member needs for management.
