Job summary

Sharon Center for Health & Rehabilitation is a Great Place to Work affiliate seeking a dedicated Social Worker to advocate for residents and support their holistic well-being in Sharon, CT. You will conduct psychosocial assessments, collaborate with families and the interdisciplinary team, and provide counseling to residents and relatives.

You will advocate for residents’ rights, help link families with community resources, and contribute to a compassionate, multidisciplinary care environment

Qualifications

• Master's degree in social work from an accredited program.
• Current licensure or certification as a Social Worker in the state of employment.
• Experience in Long-Term Care or geriatrics preferred.
• Strong assessment, counseling and crisis-intervention skills.
• Excellent communication and interpersonal abilities.

Responsibilities

• Advocate for residents' well-being and holistic needs.
• Conduct psychosocial assessments to identify needs, strengths and preferences.
• Collaborate with families and interdisciplinary team on care plans.
• Provide counseling and support for coping, transitions, and decision-making.
• Advocate for residents' rights and preferences in care planning.
• Connect residents and families with community resources and services.
