Responsibilities

• Provide case management services to foster parents and children in the Treatment Foster Care Program.
• Develop goal-oriented treatment plans within 30 days of referral.
• Prepare reports and reviews per Service Manual guidelines.
• Maintain client records, logs, and documentation.
• Coordinate with foster parents, social workers, and other providers.
• Attend meetings and participate in planning, admission, treatment, and discharge.
• Complete ongoing training and stay knowledgeable about community resources.
