Job summary

Public Partnerships LLC is seeking a Support Coordinator in New Jersey to educate, guide, and support Medicaid participants in self-directed care programs. You will visit participants in their homes up to 60 miles away, assess home safety, and help them manage employer responsibilities while coordinating with a field team from a home office.

The role requires strong communication, reliability, and comfort with Microsoft Office and CRM tools.

Qualifications

• Must reside in New Jersey and be willing to travel to assigned counties.
• Experience working with vulnerable populations is a plus.
• Strong communication and organizational skills required.

Responsibilities

• Visit participants in their homes (travel up to 60 miles) to provide guidance and education.
• Assess home environments and address safety concerns.
• Help participants understand and manage employer responsibilities and care plans.
• Organize and manage caseload to maximize impact.
• Collaborate with a dedicated team while working independently in the field.
