National Youth Advocate Program in Indiana is seeking a Family Service Coordinator. You will provide in-home advocacy, transportation, parenting education, counseling, and referrals to youth and families, traveling daily to deliver community-based services.

Qualifications

• Bachelor's degree in Social Work or related Human Services field.
• 2 years directly applicable youth/family service experience preferred.
• Flexibility to work non-traditional hours and travel as needed.
• Valid driver's license and reliable transportation required.

Responsibilities

• Provide in-home advocacy, transportation, education, counseling and referrals.
• Travel daily to serve youth and families in community settings.
• Complete documentation and trainings in a timely manner.
• Coordinate services across mental health, medical, education and social needs.
• Maintain strong relationships with foster parents and families.
• Support reunification and stabilization efforts per case plan.
• Provide safety assessment, crisis response and behavioral stabilization as required.
