Job summary

National Youth Advocate Program in Jasper, IN is seeking an experienced Family Service Coordinator to advocate for youth and families, coordinate services, and help them live safely in the community.

You will work with youth, foster parents, and partners, traveling as needed and handling documentation, outcomes, and collaboration across agencies.

Qualifications

  • Bachelor's degree in Social Work or related field from an accredited institution.
  • 2 years of indirect service experience with youth and families preferred.
  • Must have flexible availability and a valid driver's license.

Responsibilities

  • Provides in-home supportive services, transportation, parenting education, counseling, and advocacy.
  • Travels daily to deliver community-based services to youth and families per contracts and regulations.
  • Completes required documentation and trainings in a timely manner.
  • Coordinates and monitors services across mental health, medical, educational, and social programs.
  • Reviews and updates youth plans to ensure progress towards goals.
  • Builds strong relationships with foster families and families of origin.

Hours: M-F, 9a-5p with evening and weekend hours required at times.

Compensation: $50,000

Position Summary

Under the general supervision of the Clinical Supervisor or designee, this position works to make a meaningful difference in the lives of the children and families served. This position works closely with youth, family, foster parents, and community partners,providing advocacyand support toward the ultimate goal of living safely and successfully in the community.

Responsibilities
  • Activelysupports,represents,andextendsthemission,vision,andvaluesoftheorganization.
  • Provides in home supportive services, transportation, parenting education, counseling, community resource referral, advocacy, and other social service interventions to the youth and family, meeting or exceeding established outcomes thresholds on behalf of persons served.
  • Travels daily, to provide community based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements.
  • Completesallrequireddocumentationinatimelymanner.
  • Completesallrequiredtrainingsinatimelymanner.
  • Coordinates and monitors services for the youth and family in the community, including but not limited to, mental health, medical, educational, psychological, vocational, and social services.
  • Regularlyreviewsandmodifiesyouthbasedplanstoassureconsistentprogressandsuccessonwrittenplangoals.
  • Establishesandmaintainsstrongrelationshipwithassignedfosterparentsandfamiliesoforigin.
  • Assists in family reunification and/or stabilization efforts with the youth’s primary family or other permanent living arrangement consistent with the youth’s case plan.
  • Providessafetyassessment,crisisresponse,andbehavioralstabilizationservicesasrequiredtoassuresafetyandstabilityforyouthserved.
  • Consistentlyachievesestablishedproductivitythresholds.
  • ParticipatesintheContinuousQualityImprovementactivitiesonamonthly/quarterlybasis.
  • Activelyparticipatesintheweeklysupervisionprocess.
Minimum Qualifications
  • Bachelor's DegreeinSocialWorkorcomparableHumanServicesfieldfromanaccreditedinstitution.
  • 2yearsofworkexperienceworkingindirectservicewithyouthandfamiliesstronglypreferred.
  • Must have flexibleavailability as needed.
  • Musthaveavaliddriver’slicense,reliabletransportation,automotiveinsurance,andagooddrivingrecord.
  • Must have car insurance requirements of 100,000/300,000 bodily injury insurance.
  • Awillingnesstoworkflexibleandnon-traditionalhoursintheserviceoffamiliesoforigin,andpersonsserved.
  • Capacitytobeflexibleandresponsivetoyouthserved,fostercaregivers,systempartners,andinternalcustomers.
  • Capacitytoremainobjectiveandprofessionalinallareasofjobfunction.
  • Demonstratestoleranceandrespectfortheideasandactionsofothers.

  • Proficient use of desktop and laptop computers, smartphones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
