ST VINCENT FAMILY SERVICES is seeking a Care Coordinator to engage with children, youth, and families facing behavioral health needs in Columbus. You will coordinate services, facilitate team meetings, and ensure safety and effective treatment planning across home, school, and community settings.

The role requires experience in children’s behavioral health and a strong ability to collaborate with diverse providers, families, and schools.

Qualifications

• Background in children's behavioral health, child welfare, or related field.
• Experience delivering community-based services to children, youth, and families.
• Three years with high school diploma; or two years with Associate/Bachelor's; or one year with Master's.

Responsibilities

• Lead care team meetings to monitor progress and ensure safety and treatment needs.
• Identify cultural factors affecting engagement and care planning.
• Coordinate family-based services in home, school, and community.
• Link families to local resources and referrals.
• Provide crisis response during On-Call rotations after hours.
• Complete required training and maintain certifications.
• Use CANS assessments to inform care planning and updates to the plan.
• Schedule visits and coordinate with schools and agencies.
