Job summary

Mannacare Health and Wellness Medical Center in Lima, Ohio seeks a licensed Nurse Practitioner to join our primary care team, providing care across the lifespan including exams, chronic disease management, preventive care, and acute diagnosis/treatment.

You will work closely with a collaborative clinical and administrative team to deliver high-quality, patient-centered care in a supportive, multi-provider practice with flexible hours.

Qualifications

• Active Ohio APRN license with FNP certification.
• Active Ohio APRN license with AGPCNP certification.
• Active Ohio APRN license with ANP certification.
• Active Ohio PA-C license with primary care experience.
• Active DEA registration or ability to obtain one.
• Experience with Tebra or comparable EHR/practice management software.
• Experience in a primary care or family medicine setting.
• Strong communication skills and comfort in a collaborative, multi-provider practice.

Responsibilities

• Provide comprehensive primary care services across the lifespan.
• Perform routine exams and preventive care.
• Manage chronic diseases and diagnose/treat acute conditions.
• Collaborate with clinical and administrative teams to ensure high‑quality patient care.

• Active Ohio Physician Assistant (PA-C) license with primary care experience
• Active DEA registration or ability to obtain one
