Easter Seals North Georgia seeks a Community Coordinator to bridge Gwinnett County child welfare partners and Georgia THRIVe, focusing on the Safe Babies Approach and Infant Toddler Court initiatives. The role involves coordinating with families, judges, and service providers, and traveling within the county for court and field visits.

You will convene stakeholders, support family goals, and coordinate services, delivering trauma-informed support and ensuring documentation and reporting meet

Qualifications

• Bachelor degree in social work, child development, sociology or related field preferred.
• Familiarity with Gwinnett County community and local resources including early intervention, medical care, education, mental health.
• Understanding of issues related to child abuse, neglect, trauma, and domestic violence.
• Experience providing trauma-informed services to families.
• Highly collaborative management style with strong organizational skills.

Responsabilités

• Convene community partners and stakeholders to fully implement the Safe Babies Approach.
• Coordinate collaboration among community stakeholders.
• Facilitate Active Community Team (ACT) meetings.
• Meet ongoing educational and training needs of local partners and stakeholders.
• Promote systems change for prenatal to three populations and their families through cross-sector collaboration.
• Recruit and enroll families into the Gwinnett THRIVe Program.
• Facilitate service coordination for program participants.
• Meet with families weekly to assist with achieving their goals and maintain regular contact.
• Facilitate Family THRIVe meetings with families, attorneys, child welfare, and local service providers.
• Attend court hearings and provide updates on family progress and challenges.
• Assist with development and maintenance of family prevention plans.
• Ensure documentation, reports, and records are completed and submitted accordingly.
• Participate in nationwide Community of Practice calls with other Community Coordinators.
• Participate in regular meetings with the Georgia THRIVe state leadership team and national Zero to Three leadership team.
• Travel within the county for court, family visits, community resources, and required program trainings.
