Job summary

Arms Acres Inc. is seeking a discharge planning clinician to coordinate comprehensive discharge planning, maintain contact with clinical teams, families, and referral agencies, and supervise interns.

The role emphasizes treatment planning, relapse prevention, and aftercare coordination within regulatory guidelines. The position involves multidisciplinary collaboration, quality improvement participation, and potential caseload management depending on facility needs.

Qualifications

• Education and/or experience as required: high school diploma or GED or bachelor’s with CASAC or MSW/LMHC or master’s in related field.
• CASAC-T accepted during orientation with CASAC exam completion within designated period.

Responsibilities

• Discharge planning for patients with ongoing communication to clinical team, families, and agencies.
• Participate as integral part of team for difficult placements.
• Review and oversee discharge plans for rehab patients, including PCP appointments.
• Provide comprehensive evaluation of patient functioning per policy.
• Follow up with AMAs and no-shows for aftercare and document progress.
