Allaire Health Services, LLC is seeking a Discharge Planner to join our dedicated team. The role focuses on coordinating discharge plans for residents transitioning from post-acute care to home and collaborating with the care team on MDS and social services.

You will maintain social service records and complete assessments while acting as a liaison between the facility and the community. We offer a supportive environment with competitive wages and benefits.

• Experience or familiarity with discharge planning processes.
• Ability to coordinate with social services and care teams.

Mansioni

• Plan and direct the discharge plan for residents transitioning from post-acute care to home.
• Participate as a team member to address Social Service sections of MDS and care plans.
• Maintain social service records.
• Complete assessments of new and current residents.
• Acts as a liaison between facility and the community.
