Job summary

Mosaic Life Care St Joseph Medical Center ARU in St. Joseph, MO, is seeking a part-time Discharge Planner to coordinate follow-up care with patients and families.

You will work with the care team to assess needs, arrange services, and ensure appropriate referrals and resources are in place for smooth transitions. The role emphasizes communication, documentation, and collaboration with healthcare professionals to promote safe and effective discharge planning in a fast-paced rehabilitation

Qualifications

• Bachelor's degree preferred or 3 years of relevant experience.
• Masters degree preferred.
• Handle With Care certification within 30 days of hire.
• Ability to work in a stressful environment with minimal supervision.

Responsibilities

• Communicates with patients, families, medical staff, and caregivers when arranging follow-up care.
• Keeps track of Medicare forms and consent forms.
• Assists with discharge planning and identifies resources (transportation, housing, healthcare, social/spiritual services).
• Participates in case reviews to evaluate case management and progress with the healthcare team.
• Interviews and assesses patients or family members and caregivers.
• Identifies and arranges needed internal and external services/interventions.
• Assists with group activities.
