Job Overview

Provides case management services to individuals in the Developmental Disabilities program in Morrow County in accordance with Oregon Administrative Rules. Coordinates services, travels within the county, and performs duties assigned by the DD Site Supervisor, DD Program Manager, or Chief Operating Officer.

Pay Grade: B9 ($27.26–$39.76 hourly, depending on experience).

Responsibilities
  • Assess eligibility of potential clients using Oregon Administrative Rules guidelines.
  • Provide case management services to all customers in preferred community settings.
  • Maintain, submit, and ensure timeliness of all records, case notes, reports, and required documentation.
  • Assist customers in acquiring services and resources to achieve goals and maintain independence.
  • Coordinate with other community partners to develop, provide, and maintain a rich array of services for clients.
  • Provide transportation for customers when required.
  • Attend local, regional, and state meetings as requested by the supervisor.
  • Act independently within the community and promote a positive image of the agency.
  • Participate in all team and agency meetings as scheduled.
  • Keep updated on all Individual Service Plan (ISP) meetings and objectives.
  • Meet all billable services contact requirements each month, unless otherwise arranged through the Executive Director.
  • Investigate reports of abuse and implement protective services for customers when required.
  • Function as a broker for services to ensure customers meet necessary contacts (e.g., psychiatrist, medical doctor, Supplemental Social Security Insurance).
  • Participate in joint case planning and coordination with other CCS employees.
  • Remain updated on all rules and Oregon Administrative Rules (OARs) related to Developmental Disabilities Services.
  • Ensure contractors and providers monitored by CCS adhere to relevant OARs.
  • Provide consultation to on‑call staff supporting individuals with developmental disabilities.
  • Input necessary information into CCS databases for clients receiving services in the DD program.
  • Assist DD Program Manager with recruitment and maintenance of Lake County advisory board members.
  • Perform other responsibilities as assigned.
Qualifications
  • Must meet minimum standards for a Service Coordinator/Case Manager as defined in Oregon Administrative Rules 411‑320‑0030 and 411‑415‑0040.
  • A bachelor's degree in behavioral science, social science, or a closely related field; OR
  • A bachelor's degree in any field and one year of human services related experience; OR
  • An associate's degree in a behavioral science, social science, or a closely related field and two years of human services related experience; OR
  • Three (3) years of human services related experience; OR
  • If a candidate does not meet the above, a variance may be requested to the Office of Developmental Disabilities.
Certifications

None required.

  • Knowledge of or ability to learn DD case management standards and apply them.
  • Knowledge of DD services, resources, and funding sources.
  • Strong verbal and written communication skills.
  • Ability to work independently and complete responsibilities in a thorough, timely manner.
  • Represent customer and agency interests favorably to the community.
  • Valid driver's license and insurable personal auto insurance for client service purposes and travel.
  • Computer literacy; ability to use word processing and other software systems.
  • Handle highly confidential information and adhere to confidentiality rules and laws.
Background Checks

Must pass all criminal history check requirements per ORS 181.536‑181.537 and OAR 410‑007‑0200 to 410‑007‑0380. Monthly checks against OIG and GSA exclusion lists and other federal/state lists.

Pre‑Hire Drug Screen

Required.

Personal Auto Insurance

Must hold valid driver’s license and personal auto insurance for vehicles used for CCS business. Proof of liability coverage of $300,000 or more bodily injury and $100,000 or more property damage required.

Physical Demands

Use of computer, phone system, typing, reaching; sit, talk, listen; stand, walk, reach, stoop, kneel, squat; lift or move up to thirty pounds; vision requirements; sustain attention during crises.

Work Environment

Tasks at office and in community; frequent travel within county and occasional out-of-area travel and overnight stays; work in homes and community settings; may encounter uneven terrain; routine exposure to everyday risks.
