Job summary

CFG Health Systems is seeking a part-time LPN for correctional health services in Bridgeton, NJ. The role involves nursing assessments, planning, and coordinated care for inmate patients, working with RNs to achieve positive patient outcomes.

You will provide direct patient care, document in SOAP format, and participate in safety protocols while maintaining professional standards and HIPAA compliance.

Qualifications

• Graduate of an accredited Practical Nursing Program.
• Current, active Licensed Practical Nurse (LPN) license in the state of practice.
• Current CPR/BLS certification.
• Knowledge of nursing principles, practices, and procedures.
• Ability to follow clinical protocols, physician orders, and regulatory standards.
• Strong communication, interpersonal, and teamwork skills.

Responsibilities

• Performs assessments of inmate patients in a variety of settings within the institution.
• Collaborates with primary care providers and other health care providers to ensure continuity, timeliness, and appropriateness of care to inmate patients.
• Coordinates and/or administers medication, therapeutic agents, and treatments to inmate patients.
• Maintains Medical Record documentation in SOAP format, problem-oriented, and corresponding to orders; ensures timely, legible, and signed documentation.
• Promotes a culture of safety for patients and employees through proper identification, reporting, documentation, and prevention.
• Maintains competency and knowledge of current standards of practice, trends, and developments in related scope of job role or practice.
• Reports reactions to treatments and medications, as well as changes in the inmates’ emotional or physical condition.
• Provides inmate patient education and/or discharge planning.
• Records all care information concisely, accurately, and completely, in a timely manner, in the appropriate format.
