Job summary

Georgia HOPE, part of the Health Connect America family, is seeking a Client Support Specialist to provide community‑based support to individuals and families in their homes and local environments. You will work closely with therapists and teams to help clients achieve individualized goals and build practical life skills.

Qualifications

• High School Diploma with at least two years of relevant experience, or a Bachelor’s degree in a behavioral health or related field
• Experience working with children, adolescents, or adults with mental health or behavioral health needs preferred
• Valid driver’s license and reliable transportation for community‑based work

Responsibilities

• Deliver community‑based support services in homes and other real‑world settings, meeting clients where they are
• Maintain regular contact with assigned clients, including in‑person and phone‑based support
• Review client history, assessments, and treatment plans to ensure services align with individualized goals
• Support clients and families in developing and achieving goals outlined in their Individualized Recovery Plan (IRP)
• Teach coping strategies, emotional regulation, and daily living skills to promote independence and symptom management
• Help clients identify triggers, build resilience, and develop appropriate behavioral responses
• Assist with school, work, and family functioning by reinforcing practical, real‑life skills
• Collaborate with therapists and community partners to ensure coordinated, whole‑person care
• Track service utilization and maintain accurate documentation in EMR

Education

Bachelor’s degree in a behavioral health or related field
