Job summary

Star of Hope is seeking a Client Case Manager I to support clients from entry to exit, guiding counseling, coaching, and teaching toward self-sufficiency. The role involves direct intervention and collaboration with program staff to serve homeless men, women and children.

Requirements include a Bachelor's in a social-services field and a valid Texas driver license; ability to drive a 12-15 passenger van; strong communication, organization, and discretion in handling sensitive information.

Qualifications

• Requires a Bachelor's degree in a social services-related field.
• 6 months to 1 year of related work experience or equivalent given education.
• Valid Texas Driver’s License with acceptable driving record and insurance.
• Ability to drive a 12-15 passenger van.
• Good reading, writing, time management and multitasking skills.
• Discretion handling confidential client and donor information.
• Ability to communicate and work with staff, clients and partners.
• Culture fit: respectful, professional, Christ-centered service.

Responsibilities

• Consult with client weekly to set goals, track progress, and assess needs.
• Develop action plans and input goals into HMIS for accountability.
• Ensure accurate entry of services and notes into HMIS within 48 hours.
• Participate in regular staff meetings.
• Refer clients to appropriate counselor or program.
• Enforce program guidelines and core values.
• Conduct monthly apartment inspections and document results.
• Participate in trainings and professional development.
• Plan and participate in client activities.
• Assist in transporting clients and maintain driver status.
