Job summary

Qualifications

Responsibilities

• Ensure the safety and well-being of children.
• Provide culturally appropriate case management and refer clients to social/health services when needed.
• Conduct monthly home visits with children and monitor placement; document interactions with families, providers, and collateral contacts.
• Transport clients and their families to medical appointments.
