Connections for the Homeless in Evanston is seeking a Case Manager to provide essential case management services to individuals and families in our Supportive Housing Program. This role involves working closely with clients to promote self-sufficiency and access necessary community resources.

The ideal candidate should have experience with homeless populations, excellent communication skills, and a valid Illinois Driver’s License. Join our team and help make a difference in our community!

Qualifications

• Experience working with the homeless and clients impacted by trauma.
• Valid Illinois Driver’s License required.
• Able to meet clients in their homes.

Responsibilities

• Provide case management and support for up to 16 clients.
• Maintain participant files and prepare required documentation.
• Advocate for participants to receive necessary services.
