Job summary

Operation PAR, Inc. seeks a dedicated Case Manager to deliver comprehensive services for adults with substance use and mental health needs. You will screen clients, motivate treatment adherence, and coordinate referrals within a multidisciplinary team.

Responsibilities include managing confidential records, documenting progress, and expanding referral networks while maintaining compliance with ethical and legal standards.

Qualifications

• Bachelor's degree in a directly related field plus four years of direct client service experience in mental health or substance abuse.
• Master's degree in counseling or a directly related field with two years of direct client service experience in mental health or substance abuse, licensed or pursuing licensure preferred.

Responsibilities

• Screening of clients to determine appropriate level of care.
• Provide Brief Interventions using Motivational Interviewing to motivate and ensure compliance to screening recommendations.
• Establish educational groups for clients to provide information regarding substance abuse.
• Protects client rights to privacy and confidentiality in records handling.
• Prepares accurate, timely client records consistent with agency standards and confidentiality rights.
