Youth Services System Inc is seeking a Case Manager in Wheeling, WV to assess, plan, coordinate, and evaluate services for assigned clients under supervision. The role requires a 40-hour workweek with some local travel and a strong focus on safety, licensing compliance, and evidence-based service delivery.

Responsibilities include developing initial and comprehensive service plans, linking clients to services, attending court hearings, and advocating for clients with courts and providers.

Formación

• Bachelor degree or 2-4 years related experience.
• Master's degree or license eligibility preferred.
• Valid driver's license required.

Responsabilidades

• Complete Initial Service Plan and Psycho-social assessment within 72 hours of intake.
• Complete comprehensive service plan within 30 days of intake.
• Link clients to needed services.
• Review comprehensive service plan every 90 days.
• Attend court hearings.
• Advocate for clients and families with court system and service providers.
• Convene MDT, IEP, and other meetings.
• Perform other duties as assigned.
