Job summary

Wyandot, Inc. is seeking a Case Manager- Supported Employment to engage clients toward competitive employment or education completion.

You will work closely with Behavioral Health, Medical Staff, families and community partners to plan, assess, place and follow through on employment opportunities for individuals with mental health needs. Responsibilities include job development, coordinator with disability services, and regular community-based activities.

Qualifications

• Bachelor’s degree required or equivalent education plus experience.
• Experience in mental health or community-based outreach preferred.
• Strong interpersonal and writing skills are essential.

Responsibilities

• Engage clients to develop individualized employment/education plans.
• Coordinate with behavioral health team, family, and partners.
• Conduct employer contacts and job development activities.
• Provide follow-along supports to maintain employment or education.
• Assist with entitlements and benefits information as needed.
• Lead groups on vocational topics and ensure cultural sensitivity.
• Maintain client confidentiality and professional standards.
