Qualifications

• Bachelor’s or AA in social work or related field with relevant experience.
• Experience in case management or counseling with vulnerable populations.
• In-person work five days a week; flexible hours as needed.

Responsibilities

• Provide case management, intake, eligibility screening, and referrals.
• Assess participant strengths, needs, and goals with supervisor collaboration.
• Develop individualized case plans with goals and strategies.
• Offer ongoing support, address barriers, and connect to resources.
• Monitor progress, adjust plans, and conduct field visits to sites.
