Job summary

Indianhead Community Action Agency, Inc. is seeking a Case Management Specialist to deliver comprehensive case management services and coordinate referrals, intake, eligibility, planning, monitoring, and evaluation for clients.

The role emphasizes data accuracy, file maintenance, and collaboration with partner organizations to meet grant requirements. Excellent communication and computer skills are essential, as is the ability to travel as needed.

  • Conduct follow-up calls/meetings with exited members monthly or quarterly as applicable.
  • Travel as required and adhere to program guidelines from funding sources.

POSITION: Case Management Specialist

REPORTS TO: Supportive Housing Coordinator

FLSA STATUS: Full Time, Non-Exempt

Program Responsibilities:

  • SSVF

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide Case management services
  • Coordinate various aspects of case management services, including referral, intake, eligibility determination, program planning, monitoring, assessment, and evaluation of clients.
  • Monitor and record clients' progress to ensure goals and objectives are met.
  • Ensure that files are set up and maintained appropriately.
  • Maintain professional relationships with organizations, county personnel, families, and service providers.
  • Direct case service allocations.
  • Participate in committees dealing with case management and related services.
  • Responsible for collecting and entering performance measure data as per grant requirements.
  • Conducts follow-up calls/meetings with exited members monthly or quarterly as applicable.
  • Ability to make decisions based on program operations and fiscal analysis.
  • Knowledge of safety precautions necessary in performing work.
  • Ability to travel as required for job-related activities.
  • Understands and articulates program guidelines from funding sources.
  • Makes recommendations on work methods, procedures, and policies.
  • Assists with developing and maintaining work plans.
  • Manages program within State/Federal regulations.
  • Other duties assigned by the Supportive Housing Coordinator.
  • Maintain education, professional expertise, and professional contacts through attendance at job-related seminars, classes, conferences, and workshops.

Requirements

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE: Knowledge of office procedures. Excellent computer skills. Some experience in preparing informational material for public distribution and photography. Some knowledge of public relations techniques and approaches. Considerable knowledge of funding resources available for programs designed to aid low-income persons, good knowledge of the decision-making process in public and private organizations dealing with the disadvantaged.

Healthcare Navigators should have experience working with social service programs or related field casework. Knowledge of case management and crisis management principles, procedures, and practices. Good personal relations skills. Demonstrates competent computer skills. Ability to present ideas effectively, in both oral and /or written form. Ability to conduct detailed analytical evaluations and studies and prepare related reports and recommendations.

COMMUNICATION SKILLS: Excellent written, oral, and interpersonal communication skills. Ability to express ideas clearly and concisely. Communicates optimistically, positively, and with honesty and openness. Maintains confidentiality of all Agency information.

MATHEMATICAL SKILLS: Manages time effectively, uses program resources wisely, can keep records organized and turns in reports in a timely/correct manner, and completes work by deadlines. Ability to calculate figures and amounts. Ability to analyze statistical data.

REASONING ABILITY: Detail-oriented, ability to multi-task and manage time well. Self-starter with a proactive attitude who can anticipate needs. Ability to effectively prioritize and execute tasks. Highly motivated, self-directed, and attentive to detail. Displays effective problem-solving skills and identifies and implements innovative practices.

OTHER SKILLS AND ABILITIES: Ability to work in a team-oriented, collaborative environment. Ability to plan, organize, prioritize, and complete detailed projects effectively with little or no supervision. Interpersonal/human relation skills. Extensive computer skills are required.

CERTIFICATES, LICENSES, REGISTRATIONS: Valid driver's license with current vehicle insurance.
