Job summary

NorthWest Senior and Disability Services in Salem, Oregon, seeks a dedicated Case Manager. This full-time, non-exempt role assesses service needs, develops care plans, coordinates in-home and community services, and promotes health, safety, independence, and quality of life.

You will work with consumers, families, providers, and community partners, document thoroughly, and ensure confidentiality. A bachelor’s degree and four years of related experience are required; driving may be required.

Qualifications

  • Bachelor's degree in social sciences or related field is required.
  • At least four years of experience in social services or equivalent.
  • Experience with consumers in social service settings preferred.
  • Successful completion of a language proficiency test.

Responsibilities

General Description

This position plays an important role in supporting older adults and individuals with disabilities by assessing service needs, developing care plans, and coordinating services that promote health, safety, independence, and quality of life. Case Managers work directly with consumers and collaborate with community partners to connect individuals with resources and services that best meet their needs.

  • Meet with consumers to assess service needs, daily living activities, and identify appropriate support options.
  • Develop individualized care plans in accordance with program requirements and consumer needs.
  • Coordinate services such as in-home care, residential services, transportation, medical supplies, and community resources.
  • Monitor consumer needs and update care plans as circumstances change.
  • Complete required assessments, documentation, and case records in Agency and State systems.
  • Work closely with Agency staff, providers, healthcare partners, and community organizations to support consumers.
  • Help consumers address environmental, medical, or social concerns and connect them with available local resources.
  • Maintain confidentiality and comply with Agency policies, privacy regulations, and mandatory reporting requirements.
  • Provide respectful and responsive customer service to consumers, providers, and community partners.
  • Communicate directly with consumers whose primary language is not English.
  • Interpret and explain forms, services, and program information in the identified language pair.
  • Translate written materials as needed.
Minimum Qualifications

A qualified applicant will have a minimum of four (4) years of equivalent combination of education and/or experience which demonstrates the knowledge, skills and abilities required. The following qualifications meeting the minimum requirements will be considered:

  • Bachelor's degree in social sciences or any related field
  • Work or volunteer experience working directly with consumers in social service type settings. Experience can be substituted for education.
  • Any combination of related education and relevant work experience equaling at least four (4) years
  • Successful completion of a language proficiency test

While not required, experience working with seniors, adults with disabilities, or other vulnerable populations is a plus.

Other Requirements

Driving for Agency Business. Positions that require driving must possess a valid driver's license and maintain an acceptable driving record. May require proof of personal automobile liability insurance in accordance with ORS Chapter 806 and use of a personal vehicle.

Background Check. Employment is contingent upon successful completion of a criminal history check. Conviction of a crime does not automatically disqualify an individual from employment. Some positions may require additional screening, including but not limited to state or national fingerprint-based background checks.

Mandatory Reporting and Confidentiality. Must comply with confidentiality requirements and mandatory reporting obligations related to vulnerable populations.

  • Knowledge of or ability to learn social service programs and resources available to older adults and individuals with disabilities.
  • Ability to assess consumer needs, gather information, and make sound service recommendations.
  • Strong organizational skills with the ability to manage multiple cases and deadlines.
  • Effective written and verbal communication with consumers, families, providers, and community partners.
  • Ability to document clearly and maintain accurate case records.
  • Problem-solving and decision-making skills in a professional service environment.
  • Ability to build working relationships and provide respectful, person-centered customer service.
Work Environment and Physical Demands
  • Work is performed both in an office setting and in the community.
  • Regular computer and telephone use.
  • Frequent interaction with consumers, providers, and community partners.
  • Occasional travel to homes, care settings, and community service locations.
  • Frequent sitting, talking, hearing, and repetitive hand movement.
  • Occasional standing, walking, reaching, bending, and lifting (up to 20 lbs).
  • Interaction with diverse populations in office and community settings, including occasional exposure to contagious illness, challenging behavior, or domestic animals.
