Job summary

County of Dodge is seeking a Case Manager I, II, or III for Community Programs in Juneau, Wisconsin. This full-time, non-exempt position works with clients, families, and community resources to develop and implement goal-oriented service plans.

Starting wage is offered with comprehensive benefits including PTO, holidays, HSA with employer contributions, life insurance, disability coverage, flex spending, and participation in the Wisconsin Retirement System. Monday–Friday 8:00am–4:30pm.

Qualifications

• Associate degree or higher in a related field with relevant work experience in case management.
• Clear background check including caregiver and criminal history.
• Bachelor’s degree in a related field; social work license or SAC/certification may be required.
• Multiple qualifications may be considered if they collectively meet the role needs.

Responsibilities

• Engage clients, families, and natural supports to resolve problems.
• Develop goal-oriented, time-limited service plans addressing identified needs.
• Protect vulnerable individuals and maintain family autonomy in the community.
• Assess crises, coordinate referrals to community resources, and follow through.
• Maintain timely, accurate records and complete required documentation.
