Job summary

Dodge County is seeking a Case Manager to work full-time in the Human Services & Health Department. The role focuses on developing service plans, coordinating resources, and protecting vulnerable clients while maintaining comprehensive records.

Positions require progress through Case Manager I-III levels, with increasing education requirements and background checks. Regular hours are Monday to Friday with a strong benefits package offered.

Qualifications

• Associate's Degree in a related human services field and two years' related work experience in case management.
• Bachelor's Degree in a related human services field; background check required.
• Master's Degree in a related field or applicable license/certification may be required depending on level.

Responsibilities

• Develop goal-oriented, time-limited service plans for clients and families.
• Protect vulnerable populations and maintain family autonomy in the community.
• Coordinate access to services and monitor case progress to ensure timely delivery.
• Maintain complete case records and documentation in required formats.
• Consult with supervisors and interdisciplinary teams on case matters.
