The position requires an ability to build trust, conduct assessments, and coordinate services across housing, healthcare, and benefits.

Qualifications

• High school diploma or GED required.
• Minimum 1 year of care coordination/case management experience.
• Commitment to non-judgmental, person-centered care.
• Ability to build trust with individuals who may be ambivalent about health and social services.
• Experience in community-based or low-threshold settings.
• Experience with Substance use, HIV/AIDS, Hepatitis C, sex workers, and homelessness is a plus.
• Knowledge of trauma-informed care and Motivational Interviewing.

Responsibilities

• Engage participants in a respectful, non-judgmental manner and build rapport to support ongoing engagement.
• Conduct screenings and assessments across housing, healthcare, benefits, employment, and social supports.
• Develop and implement individualized service plans.
• Use Motivational Interviewing and risk reduction strategies.
• Coordinate referrals and linkages to internal programs and external providers.
• Support participants in navigating health insurance and public benefits.
• Facilitate access to services via scheduling and warm handoffs.
• Maintain accurate documentation of interactions, referrals, and outcomes.
• Participate in outreach and quality improvement initiatives.
