Job summary

Catholic Charities Eastern Washington is seeking an Onsite Supportive Housing Services (OSHS) team member to support PSH program clients through training, resource connections and wrap-around service coordination. You will collaborate with staff to stabilize clients and improve health-related outcomes across several buildings in Spokane.

The role emphasizes care coordination, documentation, and partnerships with healthcare providers while adhering to HIPAA and Catholic Social Teaching.

Qualifications

• Master’s degree in social work (MSW), Counseling, Public Health (MPH), Nursing, or related medical program, or scheduled to graduate within six months.
• In-state certification through Washington State Department of Health (SUDP or equivalent) is preferred.
• Master’s degree plus three years of direct service in housing/healthcare is acceptable.
• Knowledge of healthcare treatment, social determinants of health, and community resources.

Responsibilities

• Coordinate care with OSHS staff to stabilize clients with healthcare challenges.
• Update and implement service plans with the ICM team.
• Maintain client records, documenting services and progress.
• Build relationships with community partners and healthcare providers.
• Coordinate with partners to ensure quality health services and outcomes.
• Provide referrals and training connections as needed.
