Job summary

Catholic Charities Eastern Washington is seeking a Care Coordinator for the Catalyst emergency housing project. You will provide supportive housing services, perform assessments, and coordinate with partner agencies to guide participants toward independence.

The role emphasizes intake and care planning, collaboration with care teams, and ensuring confidentiality under HIPAA while adhering to agency standards and mission-driven service.

Qualifications

• AA degree with 4+ years experience or BA in Psychology or Social Work.
• Knowledge of community services and a minimum of two years' direct treatment experience in mental health or poverty.
• Must have a valid Driver’s License and ability to drive for work use.

Responsibilities

• Provide supportive housing services according to professional ethical conduct guidelines.
• Assist households with finding housing in the community that meets their specifications.
• Act as landlord liaison during housing search and manage background checks, applications, and leases.
