Job summary

UC Irvine Health seeks a care coordination professional to work with case managers, patients and payors to coordinate services for an episode of care. You will promote efficient resource use and assist with discharge planning, referrals, and follow-up appointments across care settings.

The role requires knowledge of government and commercial insurance plans, excellent English communication, and experience with billing software.

Qualifications

• Minimum of 2 years experience in a hospital or medical office setting.
• Extensive knowledge of Government and Commercial Insurance Plans.
• Experience with registration and/or billing computer software programs.
• Excellent written and verbal communication in English.

Responsibilities

• Coordinate care services for assigned patients for an episode of illness or treatment.
• Assist with discharge planning and resource coordination.
• Verify and authorize insurance; obtain necessary resources; send referrals.
• Schedule follow-up medical appointments; document activities in required systems.
